Get to Know All the Services Offered at A Squared Primary Care

Annual Comprehensive Preventative Health Assessment and Exam (services included in Annual Fee):

  • EKG/Spirometry;
  • Comprehensive lab panel (includes CBC, sedimentation rate, biochemical profile, glucose, renal function, liver panel, iron studies, lipid panel, thyroid panel with TSH, cardio CRP, PSA for men, PAP for women, as per current guidelines);
  • Vision;
  • Stool cards as medically indicated;
  • Urinalysis;
  • Nutrition and exercise counseling;
  • Immunization recommendation;
  • Formulation of a personalized preventive medicine plan;
  • Body fat analysis (unless the patient has a pacemaker or similar electrical equipment);
  • Waist circumference measurement with an assessment of risks;
  • 10-year cardiovascular risk assessment;
  • Mini-mental exam; and
  • Calculation of target heart rate for exercise.

Additional Medical Care

  • Administration of:
  • Routine immunizations, e.g., flu vaccine, pneumococcal vaccine, meningitis vaccine, and tetanus vaccination as appropriate (cost of vaccines charged separately);
  • Other vaccines, e.g., yellow fever, shingles, Gardasil, etc. (cost of vaccines charged separately); and
  • Necessary travel vaccines (cost of vaccines charged separately).
  • 6 blood draws & urine collections (laboratory samples will be drawn at our office and sent to your insurance company’s approved lab for testing; additional draws/collections at $10/each).
  • Enhanced preventative care services such as diet, nutrition, and exercise counseling.
  • Telephone consultation with emergency room providers, as necessary and appropriate.
  • Review of tests and consults from other providers as they become available.
  • Discussion of complementary medicine & treatment options.
  • Blood typing (if not known).
  • 5 office visits per year (additional office visits at $90 to $150/each, depending on the extent of the visit.):
  • Routine tests and procedures;
  • Sick visits;
  • Blood pressure checks; and
  • Weigh-ins.
  • House calls and nursing home visits, when clinically indicated and within a 3-mile radius of the Practice’s office :
  • Each house call/nursing home visit counts as 2 office visits;
  • Additional house calls/nursing home visits at $150/each.
  • Video visits through a secure patient portal:
  • Each video visit counts as 1 office visit;
  • Additional video visits at $50/each.
  • Telephone consultations in the event of hospitalization. (Dr. Agrawal will decide on an individual basis whether a hospital visit is appropriate. If so, each hospital visit will count as 2 office visits; additional hospital visits at $150/each.)

Coordination of Care

  • Referrals to appropriate specialists and facilities.
  • Assistance in scheduling appointments with other physicians and facilities for tests and consultations.
  • Coordination of your care with specialists and facilities.
  • Coordination and monitoring of hospital admissions and inpatient care.
  • Coordination of rehabilitation care.

Administrative & Access

  • Secure, encrypted email communications through Virtru with Dr. Agrawal for appointment requests, prescription refills, and lab & diagnostic test results. (Except during vacations /continuing medical education, where coverage will be provided by a qualified colleague.)
  • Direct access to Dr. Agrawal for appointment requests, prescription refills, lab and diagnostic test results. (Except during vacations /continuing medical education, where coverage will be provided by a qualified colleague.)
  • Secure patient portal accessible via computer, smart phone, or tablet, providing access to your personal health information, direct communications with Dr. Agrawal, appointment requests, prescription refills, and lab & diagnostic test results.
  • Completion of:
  • Pre-authorization forms for medical services and prescription medications (except where an in-plan physician referral is required); and
  • Medical history forms and medical necessity forms, including driver’s, school, and insurance (remotely when feasible).
  • Office announcements and reminders (e.g., arrival of flu vaccines, Dr. Agrawal’s vacation schedule) emailed directly to you.
  • Extended patient appointment hours (from 7:30 a.m. to 3:30 p.m. plus one evening per week).
  • Total number of patients in practice limited in order to permit more personalized care and service.
  • Wait times for scheduled visits of less than 15 minutes (except in the event of an unanticipated patient emergency).
  • Regular office appointments available same day or next day; sick visits available within 24 hours on weekdays.
  • Availability of discounted:
  • Exercise classes;
  • Meditation & mindfulness classes;
  • Education classes (such as self-defense and nutrition); and
  • Physician-recommended products & supplements.
  • Complimentary coffee, tea, and water at the Practice’s office.
